Custom Design Terms of Service Agreement (must agree to proceed with this form): 

1. By interacting with this web form (https://madpropro.com also known as "MPP"; and/or https://propstandsons.com also known as "PS"), I have hereby initiated a transaction(s) with MPP and/or PS of my own will and with implicit intent for tasking MPP and/or PS with services to include, but not limited to: graphic design, art, painting, merchandising, product development, website design, garment decoration/embellishment, and/or other creative deliverables.
2. I hereby represent and warrant Intellectual Property Rights to the "Token ID" of the "Contract Address" which I submit in this transaction. I certify that I own the full and exclusive right to grant or otherwise permit MPP and/or PS to access and to use the tokenized asset(s) (referred to as the "Token ID", the child asset of "Contract Address"), and/or associated metadata as set forth on this Agreement, and that I am aware of no claims by any third parties adverse to any of such property rights.
